Comments:
An unannounced renewal study was conducted today. All rooms are in ratio during the inspection. Board Officer files are missing some information. Your application for renewal cannot be submitted for approval until the background searches have been reviewed. 2 staff and 5 children's records were reviewed. See violation notice for missing items. Children were observed during diapering, feeding, playing outdoors. The playground was not in use during today's inspection. Standard #: 22VAC40-185-70-A
Description: Based upon a review of records, one staff record is incomplete. Evidence: Staff #1 does not have documentation of 2 references on file. (Date of Hire: 5/21/2018)

Plan of Correction: I will have two references completed for the staff member.

Standard #: 22VAC40-185-340-A
Description: Based upon a review of medication, the center did not ensure care and protection for one child. Evidence: Child #1's has an expired Epi-pen on site. (1/2018)

Plan of Correction: Parent was notified during the inspection to bring in a new Epi-pen, and a new form.

Standard #: 22VAC40-185-550-D
Description: Based upon a review of emergency drills, only one of the two required Shelter-In-Place drills was documented within the past year.

Plan of Correction: We will make sure that the drills are documented as they are conducted. The drills was done, just not documented.

Standard #: 22VAC40-191-40-D-1-A
Description: Based upon a review of records, Applicants do not have completed Central Registry Searches (CPS) on file. Evidence: *Two Board Officers do not have completed CPS searches on file. (Adult 1, Adult 2). *One Board Officer does not have a completed Sworn Disclosure Statement (SDS) on file. (Adult 2)

Plan of Correction: I have already e-mailed the information to the people. The CPS searches were sent, and we will follow up with Richmond to see where the results are.

Description: Based upon a review of records, one staff record does not contain documentation of a completed Central Registry Search (CPS) within 30 days of employment. Evidence: Staff #1 (DOH 5/21/18) does not have a completed search on file.

Plan of Correction: It was submitted. We will follow up with Richmond to see where the results are.
