22VAC40-185 STAFFING AND SUPERVISION.
A joint investigation was conducted on 6/27/2018 from 12:00 p.m. to 1:15 p.m. with the local agency to investigate a complaint regarding a forbidden action. Interviews were conducted, staff records, children's records, video recordings and documentation were reviewed. Part of the complaint regarding forbidden actions was determined to be valid.
Standard #: 22VAC40-185-340-A Complaint related: Yes Description: Based on interviews and review of video footage it was determined that staff failed to ensure care, protection, and guidance to children in care. Evidence: On 6/27/2018 at approximately 10:00 a.m. children in the preschool classroom were observed sitting at tables, some children were observed completing worksheets while other children who had already completed their worksheets were observed restlessly waiting at the tables without an activity. The children were not permitted to leave the table until all children had completed their worksheet. Between 10:04 a.m. and 10:06 am., Staff A can be seen roughly handling two children at the tables in the classroom. This incident occurred in front of the other children (ages 3 - 4 years) in the classroom. Plan of Correction: All staff will participate in training provided by The Virginia Childcare Network to cover behavioral guidance and classroom management. Virginia childcare Network will support us in a 3-prong approach: Observation of classrooms, seminar training and follow-up support
Standard #: 22VAC40-185-410-1 Complaint related: Yes Description: Based on interviews and review of video footage it was determined that staff roughly handled and struck a child which is a forbidden action. Evidence: On 06/27/2018 in the Pre-School classroom at approximately 10:04 a.m., Staff A was standing next to Child A (Age 3) who was seated at the table. Staff A can be seen forcibly grabbing Child A's face, striking the child on the head three times, followed by forcibly grabbing the child's arm and shoulder. Staff A is seen at 10:06 a.m. forcibly pushing Child B (Age 3) into a chair followed by forcibly pushing the child's head down onto the table. Plan of Correction: All staff will complete the DSS online training CWSE5691 - Child Abuse & Neglect: Recognizing, Reporting, and Responding for Educators. Additionally, training on forbidden actions conducted by management.
