Discussed with center management to make sure that fingerprint results are obtained before new staff is hired and to maintain complete staff records.
An unannounced complaint inspection was conducted in response to a complaint that was received by the licensing office on 7/3/18 related o staff records. Interviews were conducted with staff relating to the allegations of center not obtaining fingerprint results for new employee and staff records were reviewed. Standard #: 22VAC40-185-70-A Complaint related: Yes Description: Based on record review and staff interview, staff record did not include the date of separation. Evidence: 1. The date of separation for Staff # 1's was not documented in her record. 2. Staff # 2 informed the licensing inspector during the inspection that Staff # 3 informed her that Staff # 1 worked until the end of May 2018, but the date of separation was not listed in the record. 3. Staff # 4 stated that the Staff # 5 should have listed the date of separation for Staff # 1 but did not do so. Plan of Correction: We will make sure that the staff records are complete and we will make sure that start and end dates are present on the file.
Standard #: 22VAC40-191-60-C-2 Complaint related: Yes Description: Based on record review and staff interview, the center did not obtain central registry findings for staff within 30 days of employment. Evidence: 1. Upon review of Staff # 1's record, it was determined that the center had submitted the search for the Virginia central registry findings on 4/10/18. 2. The center did not obtain results for Staff # 1's central registry within 30 days of employment. 3. Staff # 2 stated that Staff # 3 informed her that Staff # 1 worked until the end of May 2018. Plan of Correction: If we do not receive results within 30 days of employment we will follow up.
Standard #: 63.2-1720.1-B-2 Complaint related: Yes Description: Based on record review and staff interview, center did not obtain fingerprinting results for employee. Evidence: 1. Staff # 1 was hired on 4/10/18. Center did not obtain fingerprinting results for Staff # 1 before she was employed. 2. Staff # 2 stated that she was told by Staff # 3 that Staff # 1 continued to work approximately till the end of May 2018. 3. Fingerprinting results were not obtained for Staff # 1 during her employment. Plan of Correction: We will make sure not to hire staff without the fingerprinting results.
