22VAC40-191 Background Checks (22VAC40-191)

The licensing inspector conducted an unannounced monitoring inspection on 07/20/2018 from 11:03am to 3:45pm. The children were observed having water play, playing on the playground, reading books, eating lunch and napping. A formal observation was completed with the twos and school age classrooms. Lunch consisted of chicken and cheese tortillas, fruit, green beans and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the mini bus. Seven children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Ten injury reports were reviewed as well as the center?s injury prevention plan, which was updated 06/15/2018. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 06/14/2018 Last shelter-in-place drill: 03/20/2018 Last fire inspection: 03/07/2018 Last health inspection: 06/20/2018 Today, the following child to staff ratios were observed: Infants - 5:2 Toddlers ? 5:2 Twos - 13:2 Twos and Threes ? 6:1 Threes - 11:2 Pre-K - 14:2 School Age - 15:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not ensure that one of six children's records contained required information. Evidence: The record of child #2 did not contain documentation of who can / can't pick up the child and the name, address and phone number of a second emergency contact. The center was able to call the parent during the inspection and obtain documentation of all persons authorized to pick up the child.

Plan of Correction: We will obtain a second emergency contact address from the parent.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: The two year old classroom had a cleaner located on top of a counter out of the reach of children.

Plan of Correction: Corrected during the inspection. We will do additional trainings on proper storage of cleaners.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center did not ensure that tables were sanitized according to standard 22VAC40-185-10 before eating lunch. Evidence: The toddler teacher sanitized the tables with disinfectant wipes before the children ate lunch. According to standard 22VAC40-185-10, the definition of ?sanitized? indicates that the surface should be sprayed with or dipped into a disinfectant solution and allowed to air dry. This allows the surface to be treated in such a way to remove bacteria and viruses. Disinfectant wipes may not be used to sanitize tables before and after meals.

Plan of Correction: Remove the Quat wipes and replace with the spray bottles.
