Comments:
An unannounced monitoring inspection was conducted on June 26, 2018 regarding a self-reported incident which was received in the Valley Licensing Office on June 21, 2018. This on-site visit was conducted from 9:45 a.m through 3:00 p.m. Five staff members and one child were interviewed along with video camera recordings. One child's record was reviewed. The violations resulting from the inspection may be found on the violation notice.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on documentation and staff interview, the licensee failed to ensure operational responsibilities are in compliance with the standards and the center?s own policies and procedures. Evidence: 1. The center does not have a written policy/procedure for identifying where children are at all times. 2. On June 21, 2018, Child #1 (age 7) ran out of the library at approximately 12:54 p.m., exited the building at approximately 12:55 p.m. and left the premises at approximately 1:24 p.m. Staff were unaware the child was missing until law enforcement called the center director at approximately 2:07 p.m. 3. Staff stated, ?We did not have a list of who was in our care that day but this week we have a plan.? 4. Staff stated, ?To be honest, we weren?t counting as much. That?s probably why we didn?t know where he was.? 5. The director confirmed there was no written policy/procedure for identifying where children are at all times except during field trips.

Plan of Correction: We have had a staff meeting since the incident and all staff knows the rules and expectations.

Standard #: 22VAC40-185-340-A
Description: Based on video review and interviews with seven staff the center staff failed to ensure the care and protection and guidance of one child. Evidence: 1. On June 21, 2018, eighteen children, seven to ten years of age, were in the care of Staff #1. 2. At approximately 12:54 p.m. Child #1 (age 7) ran out of the library and down the hall to exit the building. 3. Staff were unaware the child had left the center until the police called at approximately 2:07 p.m. 4. Child #1 walked approximately six tenths of a mile away from the center and was found by law enforcement at approximately 1:44 p.m. walking along a four lane highway.

Plan of Correction: Staff is responsible to keep a list of name sand the number of how many kids they have in their group. Staff is responsible to let the front desk and director know of their whereabouts and the total number of kids in their group. Staff is responsible to follow the schedule and be prepared to be in charge of different groups throughout the day as long as they have a list of names, know where the kids are at all times along with the total number in the group. Staff is responsible to re-count anytime we rotate from one area to another, switch from one group to another, or if a child leaves the group and if a child has been added to the group. Everyone is responsible to communicate this with the front desk person as well. Staff is responsible to hold kids accountable for asking permission anytime they are to leave the YDPL?s presence, go to the bathroom, get a drink of water, go from outside to inside, down the hallway, etc. There is always a person at the front desk that is in constant communication with everyone. The front desk person is responsible to know how many kids are in each age group and which YDPL is responsible for each age group. If the front desk person needs to leave the desk at any time, she is to communicate that with everyone and then also communicate with everyone when she returns. Staff and the front desk person know thatno kids should be sent down if no one is at the desk to watch them.
