Comments:
An unannounced monitoring inspection was completed on 7/25/18. There were 51 children present. Five children's records and five staff records were reviewed. The center does not administer medications. The inspector arrived at the center at 12:45 pm and departed at 4:30 pm. The children were observed during computer times, during game time, during gym times, and during afternoon snack. Arrivals and departures were also observed. There was discussion with the director and the administrator about the following: the requirements for staff reference checks prior to hire, background check changes.

Violations:
Standard #: 22VAC40-185-560-F
Description: Based on observation, the center failed to ensure that the menu for meals and snacks provided by the center reflected the actual food served. Evidence: 1. The menu posted for the week showed pudding and juice for the snack on the date of the inspection. The afternoon snack was observed and wheat thins and juice were served. The menu was not changed to reflect the substitution.

Plan of Correction: The menu will be changed any time a substitution is made.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the findings of the most recent inspection of the facility were not posted. Evidence: 1. The "Violation Notice" which reflect the findings from inspections was posted from an inspection July 2017. The last inspection with findings was a renewal inspection and was conducted January 2018. These findings were not posted.

Plan of Correction: The director will ensure that the "Violation Notice" from this inspection is posted today.

Standard #: 63.2-1721.1-B-2
Description: Based on record review the center failed to ensure that fingerprints as a part of the background check were submitted and completed prior to the first day of employment. Evidence: 1.Staff #2 and #3 did not have fingerprints completed prior to their first day of employment. Staff #2 date of employment was 6/28/18 and the staff person's fingerprint letter was completed 7/3/18. Staff #3 date of employment was 4/18/18 and the staff person's fingerprint letter was completed 4/27/18.

Plan of Correction: The director will make sure that fingerprints are completed prior to employment for all staff.
