Comments:
A Subsidy Health and Safety inspection was conducted on 7/25/18 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed on 7/25/18. There were 51 children present. Five children's records and five staff records were reviewed. The center does not administer medications. The inspector arrived at the center at 12:45 pm and departed at 4:30 pm. The children were observed during computer times, during game time, during gym times, and during afternoon snack. Arrivals and departures were also observed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-012-B
Description: Based on record review and interview, the center failed to ensure that within the first 90 days of employment that staff completed the Virginia Preservice Training for Child Care Staff. Evidence: 1. Staff #1, #3, #4 had not completed the Virginia Preservice Training. The staff hire dates were: 2/10/15, 4/18/18, 4/3/17. 2. The director verified that the training had not been completed by staff.

Plan of Correction: Staff will begin the training immediately and complete by August 10, 2018.

Standard #: VENDSUB-000-012-E
Description: Based on record review, the center failed to ensure that all staff providing care to children have first aid and CPR certification within 30 days of employment. Evidence: 1. Staff #3 did not have first aid certification within 30 days of employment. Staff #3 was employed 4/18/18 and had CPR certification dated 6/17. 2. Staff #4 did not have first aid and CPR certification within 30 days of employment. Staff#4 was employed 4/3/17 and had first and CPR certification dated 10/17.

Plan of Correction: The director contacted the trainer during the inspection and set up a date to complete the certification training for the staff.
