A Subsidy Health and Safety inspection was conducted on 7/24/18 from 10;0am to 12:45pm to review supplemental health and safety requirements. During the inspection there were 116 children ages five years old to twelve years old in care with 11 staff. Children were observed participating in various activities in the classrooms and gyms, and eating lunch. Records were reviewed for ten children while at the facility. The records for ten staff were reviewed at the central office on 7/24/18. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-005-B Description: Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The Licensing Inspector reviewed the attendance records for the facility, and the attendance records did not have the arrival documented for child #10 on the day of the inspection, 7/24/18. Child #10 was present during the inspection. 2. Administrative staff confirmed that there was not a written record of daily attendance that documented the arrival and departure of each child in care as it occurs. Plan of Correction: The vendor responded: Staff will be re-trained on attendance procedures to ensure that arrival and departure of each child is documented on the attendance sheet. Center management will double check the attendance record daily to make sure it is complete.
