22VAC40-191 Background Checks (22VAC40-191)
An unannounced inspection was completed on 07/25/2018 from 9:30 A.M.-1:30 P.M. At the time of the inspection 67 children were in care with eight staff and two administrators present. Seven children records, seven staff records, five medications, five injury reports, emergency evacuations, first aid kits, emergency supplies, playground, and posted required information were reviewed. Children were observed during swim time, outside playtime, A.M. snack, handwashing, doing arts and crafts making bug hats and butterfly masks, transitions time, lunchtime and naptime. Standard #: 22VAC40-185-160-C Description: Based on review of staff records, the center failed to obtain an updated tuberculosis screening at least every two years. Evidence: 1. The most recent tuberculosis screening for Staff Member #1 was completed on 02/08/2016. 2. Staff verified that an updated tuberculosis screening had not been obtained. Plan of Correction: The staff member will make an appointment and get a current tuberculosis screening before the end of the month.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to maintain all equipment in a safe and operable condition. Evidence: 1. On the Upper Playground the climbing units have peeling paint, and some of the pieces of wood on the structure are splintered or cracked. 2. On the Lower Playgrounds the climbing units and wooden structures have paint peeling, and some of the pieces of wood on the structures are splintered and cracked. 3. Staff verified that the structures had peeling paint and some of the wood was splintered and cracked and in need of repair. Plan of Correction: The Administration is currently trying to locate someone to complete this work in replacing the wooden pieces that need to be replaced and then sanding and painting the structures.
Standard #: 22VAC40-185-330-B Description: Based on observation, the center failed to ensure that all playground equipment with moving parts or climbing apparatus have the required amount of 6 inches depth of resilient surfacing. Evidence: 1. On the Upper Playground in the fall zone for the green slide the mulch had decomposed and washed away. 2. On both of the Lower Playgrounds around the structures with the yellow slides the mulch had decomposed and washed away in the fall zone. 3. On the Upper and Lower Playground there was crab grass growing up through the mulch. 4. Staff verified that there there was not enough resilient surfacing of the playgrounds and that crab grass was growing up in the mulch. Plan of Correction: The center is closed for a week in August and plan to use this week to have playgrounds mulched and the crab grass sprayed down.
