22VAC40-191 Background Checks (22VAC40-191)

The licensing inspector conducted an unannounced monitoring inspection on 07/26/2018 from 10:11am to 2:45pm. The children were observed playing with toys and games, singing songs, and eating lunch. A formal observation was completed with the preschool and school age classrooms. Lunch consisted of fish sticks, fruit, green beans, bread and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and the playground was inspected today as well as the van. Five children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Eleven injury reports were reviewed as well as the center?s injury prevention plan, which was updated 01/2018. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 06/29/2018 Last shelter-in-place drill: 06/29/2018 Last fire inspection: 10/12/2017 Last health inspection: 06/04/2018 Today, the following child to staff ratios were observed: Infants - 3:1 Toddlers ? 3:1 Preschool (Twos, Threes and Fours) ? 10:2 School Age - 13:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-A
Description: Based on record review, the center did not ensure that one of five staff records contained documentation of a negative tuberculosis screening no later than 21 days after employment. Evidence: The record of staff #3 (start date: 12/11/17) contained a TB screening dated 1/22/18. Documentation of a negative tuberculosis screening must be obtained no later than 21 days after employment.

Plan of Correction: Prior to employees start date I will require a TB screening.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: The preschool classroom had cleaners located in an unlocked file cabinet. The school age classroom had a cleaner located in an unlocked drawer within the reach of school age children.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-570-E
Description: Based on observation, the center did not ensure that prepared infant formula was dated and labeled with the child's name. Evidence: There were three prepared infant bottles in the centers refrigerator. Two bottles were labeled with a child's name but did not have a date. One bottle was not dated or labeled with the child's name.
