Discussion of a self-reported incident that occurred on July 18, 2018. A discussion was also held with regards to the needs of Child #1 and the possibility of increased supervision in the form of an additional staff person in the preschool room.

The following inspection took place between the hours of 2:30 pm through 4:30 pm. Today, I met with the staff of the Goddard School located in Leesburg, Virginia. This unannounced Non-mandated Monitoring Inspection (NMI) was conducted as a result of an incident reported by the center to the Division of Licensing. Standard #: 22VAC40-185-40-E Description: Based on review and interview, the licensee did not ensure that all activities were in compliance with the center's own policies and procedures as required by these standards. Evidence - 1. The Director stated (during an interview conducted on the date of inspection - 07/23/2018) that on Wednesday, July 18, 2018, the Preschool classroom was out on the center playground participating in water day. At approximately 10:45 am, the preschool class lined up and returned inside the building. 2. Staff #2 stated (during an interview conducted on the date of inspection) that the name to face steps in accounting for all children during transition from one area (outside) to another (back inside the building) was not conducted by herself or Staff #1. 3. Child #1 (3 years of age) was left out on the playground. 4. Staff 3 stated (during an interview conducted on the date of inspection) that as her class was entering the playground, she witnessed the Preschool class go back inside. Staff #3 then saw Child #1 go from the playground and knock on the door, where he was let back in by Staff #1. While Child #1 was not accounted for by Staff #1 and Staff #2, Child #1 was never left unsupervised. 5. Based on the Goddard Systems, Inc. 2018 QA review, it states under CR.460 - "Teacher must vocalize child's name, look at the child and refer to attendance. This practice occurs every time a child moves to a new location. . ." 6. Based on a training log dated April 19, 2018, both Staff #1, and Staff #2 were trained on the steps of "Name 2 Face Transitions." Plan of Correction: Administrative action was taken with both Staff #1 and Staff #2. Both Staff #1 and Staff #2 were retrained with regards to supervision and the steps for name to face. In addition, all other staff shall be retrained with regards to name to face during the next staff meeting.
