Description: Based on review of eight children's records and an interview with the director, it was determined that the facility does not have a written allergy action care plan for each child diagnosed with a food allergy, to include instructions from a physician regarding the steps to take in the event of an allergic reaction. Evidence: Child #8 did not have documentation of a written allergy action plan for a diagnosed food allergy.
