Technical Assistance:
Review Standards 210 regarding Program Leader Qualifications. Risk Assessment was also discussed.

An unannounced modification inspection was conducted from 2:35-4:35 PM. There were 59 children ( 4years- 13 years) present with 9 staff actively supervising the children in different activities. The Inspector reviewed procedures, evacuation plans. medication procedures, supervision, records, and ratios. The program has been operating since the last week in June, 2018. Standard #: 22VAC40-185-160-A
Description: Based on record review, the center did not obtain documentation of a negative TB screening for staff within 21 days of employment. Evidence: 1. Staff 1, 3, and 5 all have been employed since June, 2018 and all three files do not contain a TB Screening.

Plan of Correction: will obtain documentation of TB Screenings

Standard #: 22VAC40-185-40-E
Description: Based on interviews and record review, the licensee did not ensure that the centers activities and services were maintained by the terms of the current license. Evidence: 1. An Aquatics Program has been operating since June, 2018 and has 70 children (4 years to 13 years) enrolled. This program is not under the current license and had not been identified as a camp during the licensing period.

Plan of Correction: moving forward, as soon as the 2018 renewal in September, aquatics and all programs will be included on the license and identified as camp during the licensing period.

Description: Based on observation, children's records are not treated confidentially. Evidence: 1. In the entrance of the Aquatics Program, the Inspector observed 4 open card board boxes labeled Children's files 2017. The open boxes contained children's enrollment information.

Plan of Correction: records were moved during renovation temporally. move records to office space immediately

Standard #: 22VAC40-185-60-A
Description: Based on record review of children's files, the center did not keep a complete record for child #2. Evidence: 1. Child #2 was missing and Emergency Contact address.

Plan of Correction: request the address for 2 nd emergency contact from the child's family

Standard #: 22VAC40-185-70-A
Description: Based on record review of 5 staff files, the center has not maintained written information to demonstrate that 3 individuals assigned as lead positions possess the experience required by the job position. Evidence: 1. Staff 1, 3, and 4 do have high school graduation verification but do not have documentation to verify they have 6 months of programmatic experience.

Plan of Correction: will document programmatic experience for all staff

Standard #: 22VAC40-185-340-D
Description: Based on interviews, observation, and record review, the program has a grouping of children in which one program lead is supervising more than 2 aides. Evidence: 1. There were 22 children observed in the pool with 4 staff and a lifeguard. There was one staff person present who was designated as a lead. Three other staff present were aides.

Plan of Correction: ensure the leads supervise no more than 2 aides

Standard #: 22VAC40-185-510-A
Description: Based on record review, the center has been giving a child medication daily without a complete authorization form. Evidence: 1. Child #2 has been enrolled in the program for over 3 weeks and prescription medication that requires daily administration. 2. The long term medication permission form contains the parents signature for staff to administer the medication but does not contain the physician's authorization.

Plan of Correction: request physicians authorization from the child's family, but campers attendance at camp ended for the summer as of 7/27/2018
