A Subsidy Health and Safety inspection was conducted on 7/30/2018 to review supplemental health and safety requirements. Today's visit was a renewal inspection. the inspection began at 9:00am and concluded at 5:15pm. There were fifty-five children present with eleven staff members and the Program Director was also present. During the inspection departure for a filed trip was observed, the mini-bus used for the field trip was boarded, infant care was observed, two medications were present and viewed along with authorizations, lunch time was observed, nap time was observed, the emergency plan for the center was viewed, nine out of seventeen staff records were reviewed, six out of eighty-eight children's records were reviewed, emergency supplies were viewed, and some policies and procedures were reviewed. Your renewal application has not been received in the Licensing office to date. Please submit your renewal application as soon as possible as you are approaching thirty days of expiration. There were violations cited today with regards to required background checks. Please provide follow-up within ten business days or no later than the close of business day on 8/9/18. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-012-B
Description: Based on the inspector's review of nine new staff records out of eighteen total staff, the center failed to ensure a portion of the staff completed Virginia Preservice Training within their first ninety days. Evidence #1: Staff member #4 with a documented hire date of 4/6/18 has not completed Preservice training. Evidence #2: Staff member #5 with a documented hire date of 3/27/18 completed Preservice on 7/5/18, more than ninety days of after the employment date.

Plan of Correction: The PD will put a system in place to ensure all staff complete Virginia Preservice within 90 days.

Standard #: VENDSUB-000-012-E
Description: Base on the inspector's review of nine new staff records out of a total of seventeen staff records, the center failed to ensure a portion of the staff complete CPR and First aid training within thirty days of their date of employment. Evidence #1: Staff member #3 with a documented date of hire of 5/17/17 did not complete CPR/FA training until 7/2018. Evidence #2: Staff member #7 with a documented date of hire of 6/11/18 has not completed CPR/FA training to date.

Plan of Correction: The PD will ensure staff complete CPR and First Aid training within thirty days of employment.

Standard #: VENDSUB-000-032-A
Description: Based on the inspector's review of required emergency drills, the center failed to conduct and document a lockdown drill on an annual basis. Evidence: The inspector reviewed the logs used to document emergency drills and there is no evidence of a lockdown drill. The requirement for a lockdown drill has been in effect for more than a year.

Plan of Correction: A lockdown drill will be conducted and documented by the end of the program week.
