63.2 Licensure and Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on July 31, 2018. There were five staff present with 48 school age children in the gymnasium and cafeteria. Children were observed playing in the gymnasium, having lunch, preparing to go swimming and talking freely amongst themselves. Five children's records were reviewed on this date. Licensing Inspector reviewed supervision, indoor areas, injury report documentation and injury prevention procedures, activities, equipment, nutrition, children's records, emergency evacuation and shelter-in-place drill documentation and procedures, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Five medications were reviewed in the center on this date. Exit interview conducted with the Recreation Supervisor.

Standard #: 22VAC40-185-70-A
Description: Based on review, the licensee did not ensure that all required information for staff was kept at the center. Evidence: During the inspection conducted on July 31, 2018, there was no documentation of the name, address, verification of age requirement, job title, and date of employment or volunteering; and name, address and telephone number of a person to be notified in an emergency for Staff #3, Staff #4 and Staff #5.

Plan of Correction: The staff members will complete the form within 24 hours.

Standard #: 22VAC40-185-550-D
Description: Based on observation and review, the licensee did not ensure that a monthly practice evacuation drill was completed. Evidence: During the inspection conducted on July 31, 2018, there was no documentation of an evacuation drill practiced in June 2018.

Plan of Correction: Will make sure to document the drills.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and inspection of the facility, the licensee did not ensure that the findings of the most recent inspection of the facility were posted. Evidence: During the inspection conducted on July 31, 2018, the findings of an inspection conducted on 1/11/18 were posted but the most recent inspection for the facility was 3/29/18.

Plan of Correction: The inspection from today will be posted.

Standard #: 63.2-1720.1-B-2
Description: Based on inspection of the facility and staff interview, the licensee did not ensure that would receiving fingerprinting results prior to employment. Evidence: During the inspection conducted on July 31, 2018, Staff #4 was observed working at the center without having obtained fingerprint results.

Plan of Correction: The staff member will go to get fingerprinted tomorrow.
