Discussed with Adminsitration deadline for Fingerprint Criminal Check and the out-state Cps for all staff who have lived in another tate in the past 5 years.
Today an unannounced monitoring visit was conducted. This visit took place in the morning between 8:15am-10:30 there were 17 children between two buildings with 7 staff within the supervision guidelines. The children were observed having circle time, singing songs, breakfast, learning alphabet, having indoor play. Positive interactions were observed during this inspection. A complete inspection of staff records, fire drill log, attendance, and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-70-A Description: Based on review of five staff records, the center did not maintain complete staff records. Evidence: Staff #1's records did not include documentation of two or more references were checked and verified. Plan of Correction: Director stated that she contacted them and left messages but no response. Will followup with staff to get new references.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center failed to cover all unused electrical outlets accessible to children under five years of age with child-resistant protective covers. Evidence: In building #2, one electrical outlet was found missing a protective cover in the toddler classroom. Plan of Correction: Staff immediately covered the outlet.
Standard #: 63.2-1720.1-B-3 Description: Based on interview with Staff and review of staff records, the center did not obtain a Out-of-state Central Registry check for staff. Evidence: 1. Staff #1 & #2 Sworn Disclosure Statement confirmed that they currently live out of state. 2. Record for Staff#1 & #2 (hired 1/18) did not have documentation of an out-of-state Central Registry check completed prior to employment. 3. Center staff confirmed that an out-of-state central registry check has not been completed for staff #1 & #2. Plan of Correction: Admin stated that the Out of state central registry checks will be completed immediately.
