63.2 Licensure and Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on August 1, 2018. Upon arrival to the facility, there were 41 children present with seven staff members. Additional staff were on site for administrative and support purposes. The following items were reviewed as a part of the inspection: indoor and outdoor areas, supervision, administration, health and safety, physical plant, required documentation, and emergency supplies, procedures and required postings. Four medications were reviewed. The children were observed playing in centers, coloring, playing outdoors and the Infants were observed being fed. Four medications were reviewed. Five children's records and five staff records were reviewed.

Violations:
Standard #: 22VAC40-185-150-B
Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child's medical report was signed by a physician, his designee, or an official of a local health department. Evidence: During the inspection conducted on August 1, 2018, the physical report for Child #4 had not been signed by a physician, his designee, or an official of a local health department.

Plan of Correction: The parent will be contacted to have the physical signed.

Standard #: 22VAC40-185-510-E
Description: Based on review, the licensee did not ensure that medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Evidence: During the inspection conducted on August 1, 2018, two of four medications were not labeled with the child's name, the dosage amount and the time or times to be given.

Plan of Correction: The parent will be notified today and if the parent does not return by Monday, August 6th, the child will be suspended until it returns.

Standard #: 22VAC40-80-120-E-1
Description: Based on observation and inspection of the facility, the licensee did not ensure that the most recently issued license was posted. Evidence: During the inspection conducted on August 1, 2018, the facility did not have the most recently issued license posted.

Plan of Correction: The recent license was posted during the inspection.
