VENDSUB-000 STAFF QUALIFICATIONS AND TRAINING
Standard #: VENDSUB-000-005-B Description: Based on observation and inspection of the facility, the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: The following was observed: 1. In the Four Year Old Classroom, there were 10 children present in the group but the written record of attendance had three children documented. 2. In the Two Year Old Classroom, there were eight children present in the group but the written record of attendance had seven children documented. 3. In the Three Year Old Classroom, there were 10 children present in the group but the written record of attendance had 11 children documented. Plan of Correction: There will be retraining on how to take attendance. The attendance records were updated while the inspector was present.
Standard #: VENDSUB-000-006-B-3 Description: Based on record review, in two of five children's records reviewed, the vendor did not ensure that all required parent contact information was documented. Evidence: The complete work addresses were not documented in the records for Child #2 and Child #3. Plan of Correction: We will ask the parents to provide the work addresses.
