Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:00 AM and departed at 2:15PM. A total of eighty-three children were in care. Morning and afternoon activities, outdoor play, swimming and lunch service were observed. Medication administration and injury records were reviewed. A sample size of eight children's records and six staff records were reviewed. Am exit meeting was conducted with the program director prior to closure of the inspection.

Standard #: 22VAC40-185-60-A
Description: Based upon review of eight children's records and staff interview, the facility has not ensured that the record kept for each child includes the names,addresses and telephone numbers for two designated persons to be contacted in an emergency when a parent cannot be reached. Evidence: 1. The record provided for child 6 did not include street addresses for the two designated persons to be contacted in an emergency when a parent cannot be reached. 2. Staff 1 verified that the addresses were not in the record ofr child 6.

Plan of Correction: The facility responded with the following: All children's records will be checked to ensure that all required information/documentation is in each record.

Description: Based upon observation and staff interview, the facility has not ensured that children are not allowed to drink or eat while walking around. Evidence: 1. A combined group of fifteen children between the ages of eight years and 12 years were playing in the gymnasium. One staff was on duty. Two boys (siblings) were observed repeatedly going to a cubby and removing food (snack crackers) from a plastic bag and eating while walking around the gym. 2. Staff 2 was asked if children are allowed to eat while playing. Staff 2 stated that he was not aware the boys were getting food from the cubby.

Plan of Correction: The facility responded with the following: All staff will be reminded to be vigilant when supervising the children to ensure that they are not eating food from their lunch bags/boxes until snack or lunch time. Children will also be reminded of rules regarding their snacks/lunches.
