Comments:
The licensing inspector conducted an unannounced monitoring inspection Monday, July 23, 2018 from 10:45am to 2:45pm. The school age children were observed making lava lamps out of water bottles. The infants and toddlers were observed playing on the floor and coloring pictures. The preschool children were observed eating lunch. The children bring their lunch from home. All classrooms and playgrounds were inspected today. The facilities mini buses were inspected today as well. Ten children?s records and ten employee records were reviewed during this inspection. The center administers medication and it was reviewed. The center's first aid kit and emergency supplies were inspected. Total enrollment today: 112 Today, the following child to staff ratios were observed: Infant and toddlers ?9:3 Twos I?12:2 Twos II-7:1 Threes I- 10:1 Threes II- 10:1 Fours- 10:1 School Age I ?36:2 School Age II-18:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 07/23/2018, the center did not ensure that eight of nine records reviewed contained the required information. EVIDENCE: 1. The record of of Child #1 did not contain documentation of any chronic physical problems, any allergies the child may have, and the name, address and phone number of two emergency contacts. 2. The records of Child #4 and Child #7 did not have a number for the place of employment listed for the child's father. 3. The record of of Child #10 did not contain documentation of any chronic physical problems. 4. The record of of Child #6 did not contain documentation of any allergies the child may have.

Plan of Correction: Will make sure parents put n/a if information doesn't apply.

Standard #: 22VAC40-185-270-A
Description: Based on observation on 07/23/2018, the center did not ensure that equipment of the center was maintained in operable condition. EVIDENCE: Two seats on Bus #1 were torn and had the foam padding exposed.

Plan of Correction: The bus is scheduled for repair on August 9, 2018.

Standard #: 22VAC40-185-520-A
Description: Based on observation on 07/23/2018, the center did not ensure that over-the-counter skin products shall not be kept or used beyond the expiration date of the product. EVIDENCE: Suncreen belonging to Child #11 was observed in the two's room. The sunscreen expired 12/16.

Plan of Correction: Will talk to mom today to get new sunscreen.

Standard #: 22VAC40-185-550-M
Description: Based on a review of documentation on 07/23/2018, the center did not ensure that the written record of children's serious and minor injuries contained the required EVIDENCE: A sample of 10 injury records were reviewed; one was missing the method in which the parent was notified, two were missing the date the parent was notified, five were missing the time the parent was notified, five were missing the future action to prevent reoccurrence, two were missing the treatment provided.

Plan of Correction: At staff meeting on 08/22/2018, will go over how to correctly complete an injury report to ensure all information is captured and recorded. Will go over with staff that submit reports prior to the meeting.

Standard #: 22VAC40-185-570-A
Description: Based on observation on 07/23/2018, the center did not ensure that when a child is placed in an infant seat the protective belt shall be fastened securely. EVIDENCE: There were six infants seated in the feeding table in the infant room. The protective belts were not on the seats.

Plan of Correction: The straps had been removed to be washed. Will make sure the straps are put back on before the children are put back in.
