The licensing inspector conducted an unannounced subsidy health and safety inspection Monday, July 23, 2018 from 10:45am to 2:45pm to review supplemental health and safety requirements. The school age children were observed making lava lamps out of water bottles. The infants and toddlers were observed playing on the floor and coloring pictures. The preschool children were observed eating lunch. The children bring their lunch from home. All classrooms and playgrounds were inspected today. The facilities mini buses were inspected today as well. Ten children?s records and ten employee records were reviewed during this inspection. The center administers medication and it was reviewed. The center's first aid kit and emergency supplies were inspected. Total enrollment today: 112 Today, the following child to staff ratios were observed: Infant and toddlers ?9:3 Twos I?12:2 Twos II-7:1 Threes I- 10:1 Threes II- 10:1 Fours- 10:1 School Age I ?36:2 School Age II-18:1 A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11 Description: Based on a review of records on 07/23/2018, the center did not ensure that a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction was obtained. EVIDENCE: 1. The written allergy care plan for Child #11 did not contain a physician's signature. 2. The record of Child #12, who has a diagnosed food allergy, did not contain a written allergy care plan. Plan of Correction: Will talk to parents or send a letter requiring them to bring in care plans within seven days.
