Comments:
An unannounced subsidy health and safety inspection (SHSI) was conducted with the center director and center staff. A licensing inspection was also conducted on this date (see that report for details). Reminder: all staff must have documentation of the mandatory VA Pre-service Training within the first 90 days of hire. All staff must have documentation of first aid and CPR within the first 30 days of hire. There are three children with diagnosed food allergies, but two are missing the required allergy action plan completed by the physician. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. Standard #: VENDSUB-000-006-B-11
Description: Based on record review and staff interview, the vendor failed to have a written allergy care plan for each child diagnosed with a food allergy, to include instructions from a physician regarding the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: there are two children with diagnosed food allergies who do not have a written allergy action plan from the physician.

Plan of Correction: Center director will request this information from the parent.

Standard #: VENDSUB-000-012-B
Description: Based on review of 30 staff training files, the facility failed to have documentation of Virginia Pre-service training on file for each staff within 90 days of hire. Evidence: One of 30 staff did not have documentation of Virginia Pre-service training on file.

Plan of Correction: Staff has completed the training and will submit the documentation to center director.

Standard #: VENDSUB-000-012-E-2
Description: Based on review of 30 staff training files, the facility failed to have documentation of current first aid training on file for each staff within 30 days of hire. Evidence: Six of 30 staff did not have documentation of first aid training on file.

Plan of Correction: Center director will schedule a first aid class this month.

Standard #: VENDSUB-000-012-E-3
Description: Based on review of 30 staff training files, the facility failed to have documentation of current CPR training on file for each staff within 30 days of hire. Evidence: Six of 30 staff did not have documentation of CPR training on file.

Plan of Correction: Center director will schedule a CPR class for staff in August.
