22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:30am-12:00pm Upon arrival there were 55 children in care with 10 staff present. A sample of 3 staff and 5 children's records were reviewed in addition to 2 medications. Children were observed playing with staff and watching television. Infants were observed playing on the floor.
Standard #: 22VAC40-185-240-D-5 Description: Based on a record review and interview with the owner, the center did not ensure there was always at least one staff member on duty that has obtained current instruction in Daily Health Observation. Evidence: Staff #1 has DHO, however works from 9am-6pm. The center is open from 6am-6pm, leaving no staff on duty with a current DHO instruction. Plan of Correction: The following corrective action was provided by the licensee during the exit interview: The owner understands DHO training is now available online. She will have several staff members take the online training to ensure someone always is on duty with the training.
Standard #: 22VAC40-185-260-A Description: Based on a record review the center did not have documentation of an annual fire inspection. Evidence: The last fire inspection was dated 04/24/17. Plan of Correction: The following corrective action was provided by the licensee during the exit interview: The owner will call the fire department and request an inspection as soon as possible.
Standard #: 22VAC40-185-270-A Description: Based on observation the center did not ensure areas and equipment of the center, inside and outside were maintained in a safe condition. Evidence: On the playground there was a plastic sand box that was cracked with jagged and sharp edges. Plan of Correction: The following corrective action was provided by the licensee during the exit interview: The sand box will be discarded.
