Violations:
Standard #: 22VAC40-185-500-A
Description: Based on observation, not all of the hand washing procedures were followed as required. Evidence: after diapering two year old children, staff placed them on the floor and did not wash the children's hands. Staff did not wash their hands between diapering three children.

Plan of Correction: Will retrain staff on diapering and handwashing procedures at the next staff meeting.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the facility failed to follow required procedures for diapering. Evidence: 1. There was no bag or liner in the foot operated trash can in the toddler room. 2. In the toddler room diapers were disposed of in a trash can that did not have a lid and was not foot operated. 3. Staff in the twos room did not clean and sanitize the diaper pad as required. Staff used a soap and water solution to clean the pad, but did not sanitize the diaper pad (sanitize is to spray the disinfectant solution and allow it to air dry) after use. Clorox Clean Up wipes were used to wipe down the diaper pad. 4. Staff did not properly dispose of diapers in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Plan of Correction: Will retrain staff on diapering procedures at next staff meeting.
