Technical Assistance:
Technical assistance was provided in the following areas of the standards: Background checks; 22VAC40-185-(3)-210 (Program leader qualifications); 22VAC40-185-(4)-330 (Play areas); 22VAC40-185-(7)-540 (Emergency supplies)

Comments:
An unannounced monitoring inspection was conducted on 8/6/18 from 11:20m - 12:45pm. During the inspection there were 10 children ages nine months old through five years old in care with three staff. Children were observed participating in various activities in the classrooms, eating lunch and during rest period. Records were reviewed for five children and five staff. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-240-D-5
Description: Based on a review of four staff records and interviews, it was determined that the facility did not ensure that there is always at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. Evidence: 1. When the Licensing Inspector arrived staff #1 and staff #2 were present with a total of nine children. There was no documentation in the records for staff #1 and staff #2 to show that either of them had completed the DHO training within the last 3 years. 2. Staff #4 who had completed the DHO training within the last 3 years arrived at approximately 12:15pm.

Plan of Correction: The facility responded: We will have additional staff complete the DHO training and ensure there is always one staff present with DHO training.

Standard #: 22VAC40-185-550-C
Description: Based on a review of documentation and interview, it was determined that the facility did not ensure that the emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: 1. The maps for emergency evacuation and shelter-in-place were posted in multiple locations within the facility. However the procedures for emergency evacuation and shelter-in-place were not posted anywhere in the facility. 2. Staff #4 confirmed that the procedures for emergency evacuation and shelter-in-place were not posted anywhere in the facility.

Plan of Correction: The facility responded: The procedures for emergency evacuation and shelter-in-place will be posted.
