The Licensing Inspector conducted an unannounced investigation on 06/24/2018 from 12:10pm - 2:00pm, in response to a complaint that was received by the licensing office on 07/10/2018 regarding staffing and supervision and staff qualifications and training. Based upon staff interviews, records reviewed, and observations made, the preponderance of evidence gathered during this investigation does not support the allegations. Description: Based on record review, the provider did not have documentation of updated tuberculosis (TB) test or screening for each staff member. Evidence: Staff#1 has a TB test/screening on file that expired on 1/8/2017. Staff members shall obtain and submit TB test results every two years.

Plan of Correction: We have sent that employee home, the staff member will bring the TB test report back to us no later than Monday next week.

Description: Based on observation, food brought from home was not labeled as required. Evidence: In the infant classroom an infant bottle that was brought from home was observed in the classroom's refrigerator labeled with the child's name but without a date. A baby food container in the same classroom was also observed labeled with the child's name written on it, but without a date.

Plan of Correction: The director has talked to the parents to remind them to label and date all food brought from home.
