Technical Assistance:
Discussed rust on tricycles and options to prevent it from occurring. Discussed teachers labeling food items from home.

Comments:
An unannounced monitoring inspection was conducted from 8:25am-12:45pm. At the time of entrance, 39 children were in care with 9 staff members present. Children were observed in free play, reading books, listening to music, playing with puzzles and infant care. The school age classroom was headed on a field trip this afternoon. The school is clean and organized. Positive interaction was observed with the teachers and children. A selection of staff and children records, medications and other documents were reviewed. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, the center failed to obtain at least every two years the results of the tuberculosis screening for a staff member. Evidence: 1. Staff #1 had a tuberculosis screening result dated 4/24/2016 and has been more than two years.

Plan of Correction: Staff #1 has obtained her negative TB test and it is on file.

Standard #: 22VAC40-185-260-A
Description: Based on review, the center did not have an annual fire inspection report from the appropriate fire official. Evidence: 1. The last fire inspection report was dated 5/18/2017.

Plan of Correction: Payment and paperwork sent and awaiting fire inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment of the center inside were not maintained in a safe and operable condition. Evidence: 1. In the Junior Kindergarten classroom, a bench that children sit on had a red pad on top of it. The pad had a large hole about the size of a quarter.

Plan of Correction: Removed the red sitting pad from the room during the visit and ordered a new one immediately.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the toddler classroom, a can of glade air freshener spray was on top of the paper towels holder near the changing table.

Plan of Correction: Removed air freshner during visit and retrained teachers on proper storage and products used in the classroom.

Standard #: 22VAC40-185-510-N
Description: Based on review of medication, medications that were not picked up by the parent within 14 days after the authorization expired were not disposed of by the center. Evidence: 1. Child #6 had medication authorization form that expired on 6/30/2018 and had not been returned to the parent or disposed of.

Plan of Correction: Medication form has been updated and is on file.
