Thank you for your time and assistance during the unannounced monitoring inspection conducted on August 7, 2018, between the hours of approximately 10:15 a.m. and 4:00 p.m. Eighty-seven (87) children were enrolled and sixty-six (66) children, in seven classrooms, were in the care of nine staff members during the on site inspection. Two additional staff members were present and able to assist, as needed. Seven children's records were reviewed and seven staff records were reviewed and/or updated. Six children's medications four children's topical ointments and the accompanying authorization forms/logs were reviewed. Safety equipment (first aid kit, flashlight and radio), posted information (license, inspection documentation, menu, daily schedules, emergency evacuation diagrams/plans and emergency telephone numbers) and facility documents (injury reports, local agency inspections and emergency evacuation and shelter-in-place drills) were observed/reviewed. The posted menu and daily schedules accurately reflected the lunch served and the activities provided on this date. Lunch included: chicken quesadillas, green beans, pineapple and milk. Parents provided lunches for the school-age children. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Activities observed on this date included: indoor play (toys, table activities and painting); departure for fieldtrip to the park and swimming pool; restroom, diapering and handwashing procedures; lunch; infant care and naptime. The program continues to maintain substantial compliance with the licensing standards; one violation was cited during this inspection. Areas of the licensing standards discussed on this date included out of state background clearances, medication procedures and storage of hazardous substances. Standard #: 63.2-1720.1-B-3 Description: Based on review/update of seven staff records and discussion with staff on August 7, 2018, the program failed to obtain the results of an out of state search of the central registry. Evidence: 1. Review of staff #3's record, on this date, determined that an out of state search of the central registry or equivalent registry maintained by another state was not obtained. The staff member lived out of state within 5 years prior to center employment. 2. Staff confirmed the lack of an out of state central registry search for staff #3. Plan of Correction: The central registry search request will be submitted.
