The licensing inspector conducted an unannounced monitoring inspection on 08/06/2018. The children were observed playing on the playground, reading, eating lunch and napping. A formal observation was completed with the preschool and school age classrooms. Lunch consisted of cheese, wheat tortillas, mangos, peas, carrots and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the mini buses. Seven children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Six injury reports were reviewed as well as the center?s injury prevention plan, which was updated 04/2018. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 07/26/2018 Last shelter-in-place drill: 04/11/2018 Last fire inspection: 04/11/2018 Last health inspection: 06/13/2018 Last lockdown drill: 05/24/2018 Today, the following child to staff ratios were observed: Infants - 8:2 Toddlers ? 4:1 Twos - 7:1 Preschool ? 16:2 Pre - Kindergarten- 17:2 School Age - 14:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not ensure that three of seven children's records contained documentation of immunizations before the child can attend the center. Evidence: 1. Child #3 (start date: 06/18/18) did not have documentation of immunizations. 2. Child #6 (start date: 7/10/17) had documentation of immunizations dated 09/07/17. 3. Child #7 (start date: 2/26/18) had documentation of immunizations dated 4/11/18.

Plan of Correction: We contacted child #3's mother and she will bring in the immunization record today. We will make sure files are reviewed monthly and updated as needed.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not ensure that one of seven children's records contained documentation of a physical examination by or under the direction of a physician. Evidence: Child #7 (start date: 2/26/18) did not have documentation of a physical examination.

Plan of Correction: We will request that child #7's parent bring in documentation of a physical examination by the end of next week.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not ensure that one of seven children's records contained documentation of all required information. Evidence: The record of child #7 (start date: 2/26/18) did not have documentation of a written agreement between the parent and the center as required by 22VAC40-185-420 E 3 and documentation of viewing proof of the child's identity and age.

Plan of Correction: We will get the child's record current by the end of today.
