Comments:
An unannounced monitoring inspection was conducted with director and assistant director to investigate a self-reported incident forbidden actions beginning at 12:33 PM and ending at 1:45. Five staff interviews, review of documentation, and class room observations were conducted. An exit meeting was conducted with the director on August 6, 2018. It was determined that the complaint was valid and a corrective action plan was developed by the center. Standard #: 22VAC40-185-410-1
Description: Based on staff interviews, a staff used the forbidden action of roughly handling a child. Evidence: Staff B reported observing staff A roughly pulling Child A by the arm.

Plan of Correction: The incident was self reported and upon internal investigation the employee was terminated. The director had one on one meetings with each teacher and a staff meeting was scheduled August 7th to retrain staff on positive guidance, positive redirection, and behavioral guidance.

Description: Based on staff interviews, a staff used the forbidden action of demeaning verbal remarks. Evidence: Staff B and C reported that they observed/heard Staff A making demeaning verbal remarks to Child A.

Plan of Correction: The incident was self reported and upon internal investigation the employee was terminated. The director had one on one meetings with each teacher and a staff meeting was scheduled August 7th to retrain staff on positive guidance and the importance of not raising your voice and using positive redirection and behavioral guidance.
