Comments:
Licensing inspector arrived at 9:30 am and departed at 11:45 am to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there were 58 children in care with 12 staff . Additional staff arrived during the inspection. Children were observed playing with legos, playing with sand table, in circle time, participating in arts and crafts, singing, and playing musical chairs. The sample size consisted of 6 children records and 6 staff records. Areas of non-compliance was reviewed with the director.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on 1 of 6 records reviewed, the licensee did not ensure that each staff record was documented with all required information. Evidence: During the inspection conducted on August 2, 2018, the following staff record did not have have the following required documentation: Staff #6 did not have references, emergency contact, experience required documentation, health information and documentation of orientation. The center director confirmed staff #6 has worked at the center for a year. During the inspection staff #6 was on the playground was alone instructing the children in a sport activity.

Plan of Correction: Going forward we will ensure that all Enrichment program instructors have a First Discovery staff member presented during the duration of any given session. Although the instructor has provided background check and private insurance during the allotted session times, in the event that a First Discovery staff member has to step away or not able to be part of the session the chiild's parent will be required sign a waiver allowing their child to be signed out and back in by that program instructor.

Standard #: 22VAC40-185-270-A
Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside shall be maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on August 2, 2018, the licensing inspector observed multiple places in the building with green peeling paint. Pictures were provided to the center.

Plan of Correction: Areas with peeling paint will be repaired with flat paint .

Standard #: 22VAC40-191-60-B
Description: Based on statement or affirmation before they are employed or provide volunteer service. Evidence: During the inspection conducted on August 2, 2018, the record for Staff #6 did not have a sworn statement or affirmation.

Plan of Correction: Going forward we will ensure that all Enrichment program instructors have a First Discovery staff member presented during the duration of any given session. Description: Based on 2 of 7 records reviewed , the licensee did not ensure staff had a central registry finding within 30 days of employment or volunteer service. Evidence: During the inspection conducted on August 03, 2018 the record for Staff #4 (date of hire 4/23/2018) and Staff #6 ( 2017) did not include documentation of central registry results.

Plan of Correction: After speaking with the central registry department on 8/2/2018 it was determined that the results were initially sent to the wrong e-mail address although we has written the correct e-mail on the form when submitting. On 8/2/2018 the results were sent to our e-mail which we have placed in the employees files in the future we will be sure that if we have not received the results in the timely matter of 30 days we will reach out to the department of central registry to determine what the issue of why it has not been received.

Standard #: 63.2-1720.1-B-2
Description: Based on 1 of 6 records reviewed and interview with program director, licensee did not ensure an Office of Background Investigation fingerprinting determination letter prior to staff beginning employment. Evidence: During a inspection on August 2, 2018, staff # 6's record (date of hire : 2017) a did not have criminal history record check or fingerprinting determination letter. Staff #6 was working at the center without obtaining eligible status results from a national fingerprint background check.
