The licensing inspector conducted an unannounced self report of incident inspection. Staff were interviewed and other documentation was reviewed. There were 2 resulting violations cited. Please contact me if you need further assistance. Thank you for your cooperation.

Standard #: 22VAC40-185-340-A Description: Based on interviews with staff and reviewing related documents the center failed to maintain compliance with this standard as required relating to an incident on a field trip. Evidence: Staff #1 stated while interviewed that child #1, 6 years old, was left inside the vehicle for approximately 30 minutes while the other children were playing at the park. Staff #1 stated that she did not know that the child did not exit the vehicle when the other children exited and headed to play at the park. Staff #1 stated that she did not do a head count to ensure that all the children, she had transported in her vehicle, had exited. Staff #1 stated she did leave the windows partially down, and that it was not as hot and humid of a day as it had been the days previously. Staff #1 and #2 stated during interviews that a child came to staff #2 approximately 5 minutes (about 28 minutes into playing at the park) prior to the time to leave asking where child #1 was, that she had been looking all over for him. At that time staff #2 reviewed the area by sight and didn't see child #1, whom she admitted is often a wanderer. She then told staff #1 of the child possibly missing. Staff #1 immediately called all the children into a group to be watched by staff #3, while staff #1 and #2 looked for child #1. Staff #1 went towards the parking lot where she had parked and found the child strapped in his car seat, with a red flushed face, sweating, and fully alert. Staff #1 helped remove the child from the vehicle and they joined the group of children. He was given water to drink, and asked if he wanted to play for a little bit, he stated yes. Staff #1-#3 stated, in writing, that the child was acting normally and had no other issues at that time, or the rest of the day. Staff #1 failed to ensure the care, protection, and guidance of child #1 during the 30 minutes the child was alone in the parked car, in the parking lot of the park which all staff stated was more busy than normal. Staff #1 did not ensure that all the children had exited her vehicle before allowing the children to go into the park to play on 6-27-18. Plan of Correction: During this field trip this incident did happen, we all here at Children's Academic Center are Mandated reporters, even if it means on our own self. The teacher and Director did report this incident to the appropriate sources. The teacher have made a "Field Trip Attendance Sheet" for each field trip that is post in each vehicle. The states the name of the child, a departure column, arrival column, departure column, arrival column, to keep up with each child at each step of the trip. Also, the children are wearing number necklaces to help with accountability. Staff shall verify that all children have been removed from the vehicle at the conclusion of any trip. Staff have been dismissed from the center.
