Comments:
The licensing inspector conducted an unannounced monitoring inspection Monday, July 30 , 2018 from 2:21pm to 5:20pm. The infants were observed resting and playing with their teacher on the floor. The toddlers were eating snack, which consisted of chips and milk. The preschool children were observed resting on cots, and the school age children were observed playing various games. All classrooms were inspected today. Eight children?s records and eight employee records were reviewed during this inspection. The center administers medication and it was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 07/11/2018 Last shelter-in-place drill: 07/10/2018 Total enrollment today: 84 Today, the following child to staff ratios were observed: Infants ?4:1 Toddlers ?5:1 Twos & Threes ?21:1(rest time ratio) Threes ?16:2(rest time ratio) Pre-K-10:1(rest time ratio) School Age ?28:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 07/30/2018, the center did not ensure that four of the eight records reviewed contained the required information. EVIDENCE: 1. The record of Child #6 did not contain the phone number for the place of employment listed for the child's mother. 2. The most recent update of the record of Child #8 was dated 07/03/2017. 3. The most recent update of the record of Child #5 was dated 02/01/2017. 4. The most recent update of the record of Child #3 was dated 09/19/2016.

Plan of Correction: Retrain all management staff on student record requirements.

Standard #: 22VAC40-185-350-D
Description: Based on observation on 07/30/2018, the center did not ensure that during the designated rest period the ratio of staff to children is doubled. EVIDENCE: The ratio for the Two and Three Year Old Preschool class during the rest time was 21 children to 1 staff.

Plan of Correction: At 12:30pm a manager will do a walk through to check ratios.

Description: Based on a review of staff records on 07/30/2018, the center did not ensure that any employee of a licensed who is involved in the day-to-day operations or who is alone with, in control of, or supervising one or more children has a search of central registry prior to the first day of employment at the facility or before 30 days of employment at the facility ends. EVIDENCE: 1. The central registry search in the record of Staff #4, whose date of hire was 04/20/2018, is dated 07/11/2018. 2. The central registry search in the record of Staff #7, whose date of hire was 05/10/2018, is dated 07/13/2018. 3. The record of Staff #4, whose date of hire was 06/25/2018, did not contain the results of a central registry search.

Plan of Correction: Have called to check on the requests, will make sure to document calls.
