Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that were received by the licensing department on July 9, 2018 regarding allegations concerning special feeding needs. The inspection was initiated and completed on July 24, 2018 from 2:00pm to 3:00pm. Interviews with staff were completed and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. See the violation notice for more details. Complaint related: Yes
Description: Based on staff interview and review of documentation on 07/24/2018, the center did not ensure that the center's activities, were maintained in compliance with the center's own policies and procedures that are required by these standards. EVIDENCE: 1. The center's policy states "Infants, will be fed formula or breast milk in accordance with parent/guardian instructions" 2. Staff #1 stated that Child #1's parent requested that Child #1 be fed every four hours. 3. Staff #1 stated that on July 6, 2018 Child #, who is five months old, was fed one time at 10:40am. 4. The infant record documentation indicates that Child #1 received one 6oz. bottle at 10:40am. 5. Staff #1 stated that Child #1 was picked up at 5:40pm on that day.

Staff #1 stated that on July 6, 2018 Child #, who is five months old, was fed one time at 10:40am. 3. The infant record documentation indicates that Child #1 received one 6oz. bottle at 10:40am. 4. Staff #1 stated that Child #1 was picked up at 5:40pm on that day.
