Description: Based on observation, areas inside the center were not maintained in clean and safe condition. Evidence: 2 areas of chipped and peeling paint - both approximately 1.5 inches in diameter - were observed on a wall beside a barrier that separates the center's main entrance from a two's classroom.

Plan of Correction: We will have it repaired today.

Standard #: 22VAC40-185-340-D
Description: Based on observations and a review of records, 1 staff identified as a program leader was observed supervising more than two aids within a classroom. Evidence: In the infant room, Staff #12 [with program leader qualifications] was observed supervising 4 staff [Staff #9, #10, #11 and #13] who each identified themselves as assistants.

Plan of Correction: We will move a lead teacher from another classroom to the infant room.
