Today an unannounced monitoring visit was conducted in the morning between 9:15am-12:45 there were 6 classrooms with a total of 76 preschoolers and 62 summer camp students with 26 staff within the supervision guidelines. The children were observed having morning science class, making soap, outdoor free play, lunch and having field day with one of the other schools. Lunch today consisted of roasted teriyaki chicken, brown rice, carrots, fruit and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, classrooms, fire drill log, menu, medications and emergency procedures were observed. Standard #: 22VAC40-185-270-A Description: Based on observation of the center, the following areas were found not being maintained in a clean, safe and operable condition. Evidence: Chipped/peeling paint and rust was found along the water fountains in two of the classrooms. Cracked/broken tile was found along the floor at entrance from the playground. The floors in all of the classrooms had black marks and dirt buildup on the floors in the three preschool classrooms. Plan of Correction: Director stated that she will county and put in a work order to replace that the floors be cleaned and the cracked tile replaced.
Standard #: 22VAC40-185-510-D Description: Based on a review of the medication authorization forms, two long-term medication authorization forms was found beyond the expiration date. Evidence: Child #1 & 2's medication authorization forms were found expired (3/18). Plan of Correction: Director stated that they will contact the parents today to get an updated form as soon as possible.
Standard #: 22VAC40-185-510-J Description: Based on review of the physical plant and medications, medications were found in a stored place however the safe locking method was not securely fastened. Evidence: Upon arrival, the medication was found in a locked cabinet however the lock was not securely fastened. Plan of Correction: Director stated that she will remind all staff to properly lock all medication in the cabinet.
