Comments:
An unannounced monitoring inspection was conducted from 3pm through 5pm. There were 14 children in care ages two through 11 years old in care with the owner/program director and one additional staff. Children's records, staff records and required postings were reviewed. Children were observed in teacher and self directed activities including nap, watching TV, restroom routines, table top activities and coloring. Violations regarding sleep, staff records and children's records were identified and appear on the violation notice.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview the center failed to ensure that each child's records contains all required information. Evidence: 1 - The record for child 1 does not contain documentation of the addresses for the emergency contacts listed. a. Child 1 has a documented first date of attendance as 1/2/18. 2 - The record for child 2 does not contain documentation of the annual information update. a. Child 2 has a first date of attendance documented as2/11/16. There is not additional documentation of the information being updated in the record of child 2. 3 - Staff 1 confirmed that the record of child 1 does not contain addresses for the emergency contacts. a. Staff 1 confirmed that that the record for child 2 does not contain documentation of annual information update.

Plan of Correction: Going forward center management will ensure that children's records contain all required information.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview the center failed to ensure that each staff record contains all required information. Evidence: 1 - The records for staff 2 and 3 do not contain documentation of the date of separation from service. 2 - Staff 1 confirmed that staff 2 and 3 are no longer employed at the center.

Plan of Correction: Going forward center management will ensure that each staff record contains the date of separation from service as required.

Standard #: 22VAC40-185-360-C
Description: Based on observation and interview the center failed to ensure that children are allowed to sleep or rest as individually needed. Evidence: 1 - Child was sitting on the floor rocking back and forwards with her eyes closed. a. The child did not open her eyes when adults were standing over her talking. 2 - When asked why child 1 did not have a cot staff 1 stated that she sleeps all the time. a. Additionally, staff 1stated if allowed child 1 would be "knocked out" sleeping from the moment she arrives and through out the day.

Plan of Correction: Going forward center management will allow all children to sleep or rest as needed.
