Comments:
An unannounced monitoring inspection was conducted today, July 18, 2018 as a follow-up to the issuance of a provisional license. Some but not all standards were reviewed during today's inspection. Upon the inspectors arrival, the toddler and two's classrooms were observed outside on the playground. Children played in the sandbox and on the climbing equipment. The pre-school class was participating in circle time. Children were filling in the missing letters on flash card of jungle animals. Some infant were napping while the others played on the floor with toys. The school age children were on a field trip to Kelray Farms. The school age children are learning about gardening this week. The facility has age and stage appropriate materials and supplies. The first aid kit and non-medical emergency supplies were inspected and found complete. All required information was posted to include the license, last inspection notice, emergency numbers, menu, evacuation maps/procedures and daily schedules. The notice of civil penalty was posted at the entrance of the facility. The inspector interacted with staff and children in care. Seven staff records and four children's records were reviewed. Currently medication is not being administered. One staff has current MAT training. Staff have current CPR, First aid and daily health observation training. The most recent evacuation drill was conducted on 07/13/2018. The two required shelter-in-place drills were completed on 04/21/2017 and 12/18/2017. Two shelter-in-place drill will need to be completed by the end of the year. Today's Ratios Observed: Cat 1: (12 months through 2 years) 3 children with 1 staff. Cat 2: (22 months through 2 years) 7 children with 1 staff. Butterflies: (3-5 years) 12 children with 2 staff. Cuddle Bugs: (Infants) 5 children with 2 staff. Violations:
Standard #: 22VAC40-185-330-F
Description: Based on observation, the facility did not provide a shady area on the playground during the months of June, July and August. Evidence: During the inspection on July 18, 2018, children were observed playing on the playground. There was no shady area for the children.

Plan of Correction: Per the owner/Director: The shade umbrellas were destroyed during a recent storm. New shade umbrellas will be purchased.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records, the facility did not obtain a central registry finding within 30 days of employment. Evidence: 1. Staff #1, hired 04/09/2018, had documentation of a central registry dated 07/18/2018, exceeding the requirement to obtain within 30 days from employment. 2. Staff #4, hired 05/14/2018, did not have documentation of a central registry finding. 3. Staff #5, hired 05/20/2018, had documentation of a central registry dated 07/13/2018, exceeding the requirement to obtain within 30 days from employment.

Plan of Correction: Per the Owner/Director: We have devised a system to ensure background checks are completed in the time frame required by the standards. We will document all correspondence with additional agencies regarding background checks.

Standard #: 63.2-1720.1-B-2
Description: Based on review of staff records, the facility did not obtain fingerprint background checks prior to employment. Evidence: Staff #1, had a hire date of 04/09/2018, which was prior to the documented date of 04/16/2018 of the fingerprint background report.

Plan of Correction: Per the Owner/Director: We have devised a system to ensure background checks are completed in the time frame required by the standards. We will document all correspondence with additional agencies regarding background checks.
