Comments:
An unannounced renewal inspection was conducted today August 15, 2018 to determine the center's compliance with licensing standards. The inspector initiated the inspection at 10:00 am and concluded the inspection at 12:45 pm. The owner/director participated with the inspection. Upon the inspectors arrival the two year old classroom was having snack. Preschool children were observed making a color book focusing on the color green. Children colored different items that are green. The infant and toddler classroom was combined during today's inspection. Children were having snack and others were on the matt playing with toys. The school age summer campers were on a field trip to the movies and the park to eat lunch. Field trip permission slips and a posting of the field trip information was reviewed. Later children were observed having lunch and preparing for nap. The preschool class did a yoga activity and had a story before nap. Staff interacted in a positive manner and participated in activities with children. Today's lunch consisted of french toast, sausage patties, carrots, oranges and milk. All areas of the facility were inspected including the playground. The facility has age and stage appropriate materials and supplies. The first aid kit and non-medical emergency supplies were inspected and found complete. All required information was posted to include the license, last inspection notice, emergency numbers, menu, evacuation maps/procedures and daily schedules. The inspector interacted with staff and children in care. Two staff records and five children's records were reviewed. Five additional staff records were reviewed as a follow up. Currently medication is not being administered. Staff have current CPR, First aid and daily health observation training. The most recent evacuation drill was conducted on 07/13/2018. The two required shelter-in-place drill for 2017 were completed on 04/21/2017 and 12/18/2017. One shelter-in-place drill for the 2018 year was documented on 03/12/2018. Today's Ratios Observed: Cat 2: (2 years) 5 children with 1 staff. Butterflies: (3-5 years) 12 children with 2 staff. Cat 1 and Cuddle Bugs: (Infants and Toddlers) 7 children with 2 staff. School age children were not in attendance during today's inspection. 6 children and 2 staff on field trip. Corrections were reviewed for violations cited during the previous inspection. There were no repeat violations cited during today's inspection. A complete renewal application has been received by the Department. Description: Based on review of children's records, the facility did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: The most recent documentation of immunization for child #5, age eleven months, was dated 11/06/2017, exceeding the requirement to obtain additional immunizations every six months.

Plan of Correction: Per the Director: I will contact the parent today and request that a copy of the updated immunizations be provided for the record.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurements, the facility did not ensure where playground equipment is provided, resilient surfacing complied with minimum safety standards. Evidence: Mulch around the climbing equipment did not meet requirements of resilient surfacing. A depth of six inches of resilient surfacing within the fall zone is required around playground equipment. The following measurements were taken: 1. 2 inches in front of small slide. 2. 4 inches on side of larger slide. 3. 2.5 inches on side of small slide. 4. 4 inches on in front of larger slide. six inches in depth resilient surfacing reconquered six inches. Four measurements were taken around the piece of climbing equipment on the playground. The measurements did not meet the six inches of resilient surfacing requirement.

Plan of Correction: Per the Owner: I will have the landscaper install additional mulch around the equipment.
