Technical Assistance:
The area of flooring that dips in the preschool classroom was discussed with the program director. The bottom of several fence pickets are beginning to crack and splinter.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:30 AM and departed at 2:15 PM. Twenty-three children were in care at nap time. Morning activities, lunch service and nap time were observed. It was reported that no medication has been administered to children and no children's injuries were reported. A sample size of six staff records and five children's records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based upon review of six staff records and staff interview, the facility has not ensured that each staff record includes al required information/documentation. Evidence: 1. The record provided for staff 2 indicated a hire date of 8/9/18. There was no documentation that two or more references as to character and reputation as well as competency were checked before employment. The same record did not include the street address for the emergency contact person or documentation that staff 2 had received the required training prior to the end of the first day on duty. Staff 2 was on duty alone in the two year old classroom during the inspection today. 2. The record provided for staff 3 did not include the date of employ. The same record did not have documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff 3 was on duty alone with the infants during the inspection today. 3. The record provided for staff 5 indicated a hire date of 8/6/18. The record did not include documentation that two or more references as to character and reputation as well as competency were checked before employment. The same record id not include the street address of the emergency contact. 4. The records provided for staff 2 and 3, both alone on duty with children during today's inspection and identified as program leaders did not include documentation to verify the six months of programmatic experience required to be program leaders. 5. Staff 1 verified that the above documentation as not in the records for staff 2, 3 and 5.

Plan of Correction: The facility responded with the following: All staff records will be completed as required. In the future, all efforts will be made to have complete files for staff prior to staff being placed on duty.

Standard #: 22VAC40-185-340-D
Description: Based upon observation and review of staff records, the facility has not ensured that in each grouping of children at least one staff member who meets the qualifications of a program leader is present. Evidence: 1. The record provided for staff 3, who was alone on duty with the infants during today's inspection, does not include evidence of the six month's experience required to meet standard 22VAC 401-85-210.A.4. 2. Staff 1 verified that the experience was not verified.

Plan of Correction: The facility responded with the following: A program leader qualified staff person was assigned to the infant classroom during the inspection. Should staff 3 have the required six months full time programmatic experience, it will be documented in the record and staff 3 will then meet requried qualifications.

Standard #: 22VAC40-185-440-A
Description: Based upon observation and staff interview, the facility has not ensured that each child is provided with an individual crib for resting or napping. Evidence: 1. There was no crib in the infant room identified for use by child 1 who, according to the record provided, is three months old and began attending on 7/12/18. 2. Staff 3, on duty in the infant classroom stated that child 1 slept in the infant seat. 3. Staff 1 stated that a crib is being purchased for child 1.

Plan of Correction: The facility responded with the following: A crib that meets current CPSC guidelines will be purchased for child 1. Children under the age of twelve months will not be accepted into care unless there is an available crib for use by the child.
