A Subsidy Health and Safety inspection was conducted on 8/15/18 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed on 8/15/18. There were 19 children present with two staff supervising. Five children's records and two staff records were reviewed. Three medications were observed for one child in care. The inspector arrived at the school at 2:45 pm and departed at 6:00 pm. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on review of the attendance records and interview with staff, the center failed to ensure that there was a written record of attendance (hard copy) that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The attendance was observed at approximately 3:30 pm. There were 19 children signed in at 3:00 or after and only 17 children present. 2. There were eight children who were signed in for morning arrival and were not signed out. 3. According to the site director, the children are not signed out in the morning and children who are expected to arrive are signed in for the day at the beginning of the program.

Plan of Correction: The site director will begin signing the children in and out when arrivals and departures occur.

Description: Based on record review and interview, the center failed to ensure that there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. There was a child in care with two separate diagnosed food allergies and medications for those allergies. There was no allergy action plan observed for the child. 2. The site director stated that there was no allergy action plan for this child's food allergies.

Plan of Correction: The site director will have the parent and physician of the child complete the allergy action plan.
