Comments:
An unannounced inspection was conducted today, July 27, 2018 in response to a complaint received by The Department of Social Service, Children's Licensing on July 26, 2018 concerning preventing the spread of disease. The inspection was initiated at 12:00 pm and concluded at 12:30 pm. An interviews was conducted with the director, two children's records were reviewed, documentation of the center's illness policy reviewed and an observation was made. Based on the information gathered during the investigation, the preponderance of evidence supports the allegation, therefore the complaint is valid. An additional violation was cited based on observation regarding supervision that was not related to the complaint. Today's Ratios: classroom #1: (16-24 months) 3 children with 1 staff. classroom #2: (24-36 months) 4 children with 1 staff. classroom #3: (3-5 years) 3 children with 1 staff classroom #4: (4-5 years) 5 children with 1 staff If you have questions regarding this inspection, please contact the inspector at 804-662-9758.

Description: Based on observation, the facility did not ensure children under 10 years of age were always within actual sight and sound supervision of staff. Evidence: 1. Upon arrival, the inspector was greeted by the director in the hallway near the side entrance. The director was moving boxes from the hallway area and toddler classroom. After informing the director of the purpose of today's inspection, the director lead the inspector to a classroom down the hall passing a classroom where child #2 (five years in age) was observed alone, sitting in a chair. 2. The director stated the child had been having a difficult time in class and was acting out. The Director also reported that she had moved the child into the other classroom to allow her time to calm down and that she (the director) had been moving in and out of the classroom. The two classrooms were side by side however their was no direct supervision of the child by a staff member.

Plan of Correction: Per the Director: II will always keep the children with me or another teacher.

Description: Based on interview, the facility did not notify parents within 24 hours of the center's having been informed of a communicable disease within the center. Evidence: The director reported she did not notify the parents of the center within 24 hours of the center's having knowledge of child #! diagnoses of strep throat, which is listed on the communicable disease list.

Plan of Correction: Per the director: I will post a note for all parents to see for any communicable disease and date it.
