An unannounced monitoring inspection was conducted from 9:30am - 1:00pm with the center director and staff. There were 72 children, ranging from three months to eleven years of age, present with 9 staff supervising. Children were observed participating in arts and crafts, structured center play, outdoor play and creating self portraits. Lunch served today is spaghetti, green beans, applesauce and milk. Seven children's records and four staff records were reviewed. The emergency escape drill log was observed and in compliance. There were no medications to review. Standard #: 22VAC40-185-130-A Description: Based on a review of seven children's records, the center did not obtain documentation that each child has received immunizations prior to the child's first attendance. Evidence: Child #6 (date of first attendance: 07/10/17) did not have documentation of immunizations in their file. Plan of Correction: A new immunization form will be given to the parents tonight and they have til Friday, 8/24/18, to complete.
Standard #: 22VAC40-185-140-A Description: Based on a review of seven children's records, the center did not obtain documentation that each child has received a physical examination before the child's attendance or within one month after attendance. Evidence: Child #6's (date of first attendance: 07/10/17) record did not have documentation of a physical examination. Plan of Correction: A new physical form will be given to the parents tonight and they have til Friday, 8/24/18, to complete.
