Discussed with Administrative staff: Monthly Assessment of accidents to see if there is a pattern to the accidents. Also discussed assuring that correct medication bottle is brought in by the parent. Also discussed AuviQ training.

An unannounced renewal inspection was conducted beginning at 9:35 and ending at 12:20. There were 56 children present, ages 7 weeks - 11 years with 10 staff supervising. Children were observed during circle time, participating in table activities, leaving for Field Trip to water park and having snacl of rice cakes, yogurt and water. Reviewed 3 accident reports 0- see Violation Notice Reviewed Emergency drills - see violation notice. Reviewed 4 medications - complete Reviewed5 staff files and 5 children's files- complete Health and Fire Inspections are current as is Liability Insurance. Standard #: 22VAC40-185-500-B Description: Based on observation, it was determined that not all diaper disposal systems in classrooms have covers/tops. Evidence. The top cover for the diaper disposal container in the infant room does not have a top. It was lost during a storm. Plan of Correction: New Diaper disposal system will be purchased.
Standard #: 22VAC40-185-550-D Description: Based on a review of emergency drills from this last year, it was determined that drills were not done every month. Evidence: There were no documented emergency escape drills for March - May. Plan of Correction: Emergency escape drills will be done monthly, shelter in place drills will be done twice a year and lockdown drill will be done once a year or per Fire Marshal's directions.
Standard #: 22VAC40-185-550-M Description: Based on a review of one age group's accidents, it was determined that not all forms were completed with requiored information. Evidence: One of 3 forms did not have time parent was notified of the accident. Plan of Correction: Director will review with all staff the correct completion of accident report forms.
