An unannounced monitoring inspection of the summer camp was conducted today from 10:30am-1:30pm. There were 70 children (3yrs-8yrs) directly supervised by 14 staff. The physical plant, swimming pool, 7 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in a cooking activity, swimming, having outdoor/indoor play time, and eating lunch. Ratios and supervision were in compliance. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not obtain updated tuberculosis (TB) test or screening for each staff member every two years. Evidence: Staff B has documentation on file of a TB test/screening that expired on 6/15/2017.

Plan of Correction: We will send the staff member to get a new one.

Standard #: 22VAC40-185-510-A
Description: Based on review of children's medications and interview, it was determined that the center has a medication on site without an authorization form. Evidence: Child A has a prescription medication on site without a medication authorization form completed and signed by the parent and the physician.

Plan of Correction: We will request an authorization form.

Standard #: 22VAC40-185-510-F
Description: Based on review of medications, prescription medication was not maintained in the original labeled container. Evidence: The prescription medication (Proair HFA) for child A was not in the original container with a prescription labeled attached.

Plan of Correction: We will ask the parents for the original medication container.

Standard #: 22VAC40-185-560-G
Description: Based on observation, it was determined that lunch boxes brought from home by the children were not labeled with the child's name and date. Evidence: In the preschool camp area, six lunch boxes were observed without a date; four of those lunch boxes were not labeled with the child's name.

Plan of Correction: We will continue to reiterate to families that they need to label all lunch boxes.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not obtain an original central registry report within 30 days of employment of a staff member. Evidence: Staff A's file does not contain a central registry report. The staff member was hired on 6/2018.

Plan of Correction: I confirmed via our internal documentation that we have received it, but I need to locate the document, and I will verify with the agency to see if we can get a verification.
