The new renewal application was reviewed. Documentation of medication administration discussed.
An unannounced monitoring inspection was conducted on 08/16/2018 from 10:15am - 2:30pm. At the time of the tour there were 60 children in care ranging in age from two years - twelve years. Seven staff were present with the children. The program director was also present during the inspection. The inspector observed morning program time which included music, reading and a visit from the Science Adventure Program. Lunch and nap was additionally reviewed. The sample size included nine children and three staff records. Medication and seven injury reports reviewed. The violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.
Standard #: 22VAC40-185-140-A Description: Based on record review and interview, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. There was no documented physical exam on file for child 1. a. Child 1 has an enrollment date of 06/18/2018 and was in care during the inspection. b. A written note was on file for child 1, from his physician, stating that he had an appointment with the physician on 08/30/2018. c. Administrative staff confirmed that she had requested a physical from the parent for child 1. 2. There was no documented physical exam on file for child 2. a. Child 2 has an enrollment date of 03/27/2017 and was in care during the inspection. b. Administrative staff confirmed that a physical exam was not on file for child 2. Plan of Correction: 1. In the future the center director will not grant any extensions to the stated policy. 2. Va School Physical form is 3 pages long. Pages 1 & 2 were in the record however, page 3 was not included in the record. In the future the center director will carefully look to be sure that all pages are included before filing in child?s record.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center failed to ensure that the diapering surface was properly sanitized after a diaper change. *"Sanitized" means that the surface is sprayed with the disinfectant solution and allowed to air dry after use of the disinfectant solution. Evidence: 1. One diaper change was observed in the two-year old class at 11:45am. The staff member sprayed the diapering table with bleach and water after completion of the diaper change. However, the disinfectant was not allowed to air dry as the staff member used a paper towel to dry the surface immediately after spraying. Plan of Correction: 1. As stated above the staff member did clean and sanitize the diapering area but let it air dry for less than required time. The staff member has been retrained to always allow sanitizing and air drying of 2 minutes. After 2 minutes if not completely dried we may use a paper towel to dry surface.
