Standard 22VAC 40-185-70.A was discussed with the program director to include addresses for emergency contacts for staff. Foods that are potential choking hazards for children aged three and under was discussed with the program director. Staff will ensure that any potential choking hazards sent by parents for children's lunches will be prepared to reduce the potential for choking. Ex: grapes, hotdogs sliced, etc.
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:30 AM and departed at 2:50 PM. One hundred twenty children were in care. Morning activities, outdoor play,lunch service and nap time were observed. Medication administration and injury records were reviewed. A sample size of ten staff records and ten children's records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.
Standard #: 22VAC40-185-160-C Description: Based upon review of ten staff records and staff interview, the facility has not ensured that results of tuberculosis screenings are obtained at least every two years. Evidence: 1. The most recent tuberculosis screening documentation in the record for staff 4 is dated 5/19/15. 2. Staff 1 verified that staff 4 has not yet obtained an updated tuberculosis screening. Plan of Correction: The facility responded with the following: All staff records will be checked to ensure that tuberculosis screening documentation is obtained at least every two years.
Standard #: 22VAC40-185-70-A Description: Based upon review of ten staff records and staff interview, the facility has not ensured that each staff record includes documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: 1. The record provided for staff 9 includes documentation of only one reference check. 2. Staff 1 verified that the record for staff 9 included documentation of only one reference check. Plan of Correction: The facility responded with the following: We will ensure that we obtain and document at least two reference checks prior to employ of new staff.
