Technical Assistance:
Discussed the replacement of one stained crib sheet.

Comments:
An unannounced monitoring inspection was conducted today. There were 39 children in care, with a total of 9 staff members assigned to their direct supervision. Children's records, staff training records, background checks, children's medications, emergency preparedness items, the physical plant, and children's nutrition were inspected today. The children were observed playing with toys, singing songs, practicing color and letter recognition, reading books, completing circle time activities, and eating lunch from home. This inspection was completed between approximately 11:00am and 12:45pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record #2 and #3 do not include documentation of annual review completed by the parent. Both of these records were most recently reviewed by the parent in 7/17.

Plan of Correction: I will create a new schedule for all parents to review their child's registration form information.

Standard #: 22VAC40-185-510-A
Description: Based on a review of children's medications, it was determined that one medication is on site without a properly completed authorization form. Evidence: One child's non-prescription medication is on site with a completed authorization form that is used for the local public schools. The wording on this form indicates that only the public school employees are authorized to administer the medication.

Plan of Correction: I will ask the parent to complete our medication authorization form.
