22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 8:45am-12:00pm. At the time of entrance, 43 children were in care with 6 staff members present. Children were observed playing with toys, listening to music, engaged in group activities and outdoor play time. The center recently updated their playground equipment. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-270-A Description: Based on observation, equipment of the center outside was not maintained in a safe and operable condition. Evidence: 1. A water table cover on the playground was rough and splintered. Plan of Correction: I will throw the table top out immediately.
Standard #: 22VAC40-185-520-C Description: Based on review, the center did not meet all requirements when administering diaper ointment or cream. Evidence: 1. In the Lady Bugs classroom, Child A had a diaper cream labeled with the child's name on it, but did not have written parent authorization to give the diaper ointment. 2. In the Lady Bugs classroom, Child B had a diaper cream labeled with the child's name on it, but did not have written parent authorization to give the diaper ointment. The form had expired on 7/18/2018. Plan of Correction: I will have everyone do new forms.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center did not obtain the central registry findings within 30 days of employment for a staff member. Evidence: 1. Staff C began employment at the center on 7/17/2018 and the center did not have the central findings within 30 days of employment. Plan of Correction: I will resend it today and scan and send it to Central Registry. I spoke with them on the phone today to follow up.
