Comments:
An unannounced renewal inspection was conducted today from 10:13am-1:45pm. There were 78 children (infants-5yrs.) supervised by 13 direct care staff. The physical plant, 5 staff records, 8 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping, transitioning to outdoor play, and engaging in music class. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. If you have any questions regarding this inspection, please contact the Licensing Inspector, Anansa Archer-Hicks at (703) 268-3295 or anansa.archer-hicks@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, 2 of 8 children's records did not contain all of the required information. Evidence: 1. Child #1 (start date: 7/3/2017) and Child #5's (start date: 10/30/2017) records did not contain documentation of proof of identification. 2. Child # 5's record did not contain documentation of a complete address or telephone number for the second emergency contact person.

Plan of Correction: We will get the missing information form the parents this afternoon and add it to the children's files.

Standard #: 22VAC40-185-500-A
Description: Based on observation, a staff member did not wash their hands with soap and running water after performing a diaper change. Evidence: Staff C (date of hire: 7/12/2018) did not wash their hands after performing a diaper change.

Plan of Correction: We provided training during the inspection to the classroom aide to remind the staff member to wash hands and not just change gloves, after performing diaper changes.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not maintain a nonabsorbent surface for diapering or changing. Evidence: The diaper changing pad in the Junior Preschool classroom had a rip in it.

Plan of Correction: Replaced the diaper changing pad during inspection.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medications, 1 of 6 medications on site had an expired medication authorization. Evidence: Child #7's (start date: 6/4/2018) cough syrup had a medication authorization form that expired 7/2/2018.

Plan of Correction: We will send the medication home with the parent at pick up. The child is no longer receiving the medication.
